Who is responsible
Elevate Aerial Physiotherapy, operating from KPole Wolverhampton, Wolverhampton. The registered data controller details and the ICO registration number are outstanding.
What this website collects
Two forms. The contact form takes your name, your email, the studio you train at if you give it, and a short description of what is going on. The studio form takes a studio name, a location, the apparatus taught and what students are asking about.
The contact form invites you to describe an injury, which is health data under UK GDPR. It is deliberately kept short and optional. Clinical detail belongs in the appointment record, not in a web form, and nothing typed into that field is ever placed into analytics.
Clinical records
Appointment booking and clinical notes are handled in Cliniko, not on this website. Retention of physiotherapy records follows Chartered Society of Physiotherapy guidance. The exact retention period and the Cliniko data processing terms are outstanding.
Analytics and cookies
Whether analytics run on this site at all is outstanding. If any are added, this page names them before they are switched on, and no analytics tool ever receives the contents of a form field.
Your rights
You can ask what is held about you, ask for it to be corrected, and ask for it to be deleted where there is no clinical or legal reason to keep it. Ask through the contact page.
